Let's talk

Book an intake, bring the program to your location, or join our network of trainers and allied health professionals.

Book your intake

We'll confirm your appointment at the nearest partner location. Please don't include detailed medical information here; we'll collect it securely at intake.

No code yet? Take the 2-minute pre-screen. It's optional, but it speeds up your intake.
Thanks! We'll be in touch within one business day to confirm your intake.
FAQ

Common questions

Is the program covered by OHIP?

No. The Performance Program is a private-pay service and is not covered by OHIP. Medication is purchased separately at a pharmacy and may be covered by your private drug plan. Some allied health services may be eligible under extended health benefits.

Do I need a referral from my family doctor?

No. The program operates independently of your relationship with your family physician. With your consent, we're happy to share your results and treatment summary with your doctor.

What is the QR code at my gym?

Partner locations display Tide Health QR signs at their entrance, exit and around the club. Scanning one opens a 2-minute pre-screen. When you finish, you get a personal intake code to bring to your Nurse Practitioner intake, so your appointment starts with your answers already in hand. The pre-screen is not a medical assessment; eligibility is always decided at intake. Your gym never sees your answers.

Who provides my care?

A licensed Nurse Practitioner performs your intake, decides whether GLP-1 therapy is appropriate, prescribes, and monitors you throughout the program, in person on clinic days and through the app between visits.

What happens at the intake?

A comprehensive assessment: medical and family history, medications, goals, baseline measurements and a full blood panel with other health markers. Nothing is prescribed until your results are reviewed and you're confirmed as a good candidate.

What if I don't qualify?

If the program isn't safe or appropriate for you, the Nurse Practitioner will explain why and discuss other options, which may include training, nutrition and allied support without medication.

Why is it a 6-month commitment?

Six months allows for safe dose titration, regular monitoring, and enough time to build the strength and nutrition habits that help results last.

How often will I see the Nurse Practitioner?

You'll have an intake, a results and start-up visit, and monthly follow-ups for the rest of the program. Clinic days start every two weeks at each location and increase with demand. You can message the care team in the app any time.

Do I have to be a member of the gym?

Policies vary by location. Some partners offer program clients a membership or day-pass arrangement; others require membership. Your partner location will confirm when you book.

What do allied health sessions cost?

$115 per session for any discipline in the network: nutrition, massage, physiotherapy, kinesiology and more. Sessions are bundled in the Complete tier or can be added to any tier.

How is my health information protected?

Your clinical record is held by Tide Health, not the gym. Trainers and allied professionals see only what you consent to share through the app. Partner staff never access your medical information.
