The 6-Month Performance Program
Medical assessment, prescribing and monitoring by a Nurse Practitioner, delivered at your gym, and backed by trainers, nutrition experts and an app built for life on a GLP-1.
Who the program is for
GLP-1 medications can be a powerful tool for metabolic health. Our job is to decide, with you, whether they're appropriate, and then make sure you get the most from them safely.

Who may not qualify
Eligibility is always decided by the Nurse Practitioner at intake. The program may not be appropriate if you are pregnant, breastfeeding or planning pregnancy, have a personal or family history of medullary thyroid carcinoma or MEN2, a history of pancreatitis, or certain other conditions and medications. Your bloodwork and history guide the decision, and if it isn't right for you, we'll say so and suggest alternatives.
The intake appointment
Your first visit is a comprehensive on-site assessment with the Nurse Practitioner to evaluate and qualify you for GLP-1 care. Nothing is prescribed until the results are in and reviewed.

If you pre-screened by scanning a QR sign at your gym, give your intake code at check-in. The Nurse Practitioner will already have your answers, so your appointment starts with your care. No code? Pre-screen now, or simply book, since it's optional.
Full bloodwork
A comprehensive panel ordered at intake, which may include:
- HbA1c, fasting glucose and insulin
- Lipid panel
- Kidney function (creatinine, eGFR) and electrolytes
- Liver enzymes
- Complete blood count
- Thyroid (TSH)
- Vitamin B12, vitamin D, iron & ferritin
Body & health markers
Baseline measurements we track all six months:
- Weight, BMI and waist circumference
- Blood pressure and resting heart rate
- Body composition (fat vs. lean mass) where available
- Strength and fitness baseline with your trainer
- Sleep, energy, appetite and mood check
History & goals
A full clinical conversation covering:
- Medical and family history, current medications
- Contraindications and risk screening
- Previous weight-management attempts
- Nutrition, training and lifestyle habits
- Your goals, and what success looks like to you
What happens, month by month
The program is a six-month commitment. That's long enough to titrate safely, build strength and establish habits that last.
Scan the QR code at your gym
Scan a Tide Health QR sign at your gym's entrance, exit or around the club and complete a 2-minute pre-screen. You'll receive a personal intake code to bring to your first appointment.

Assessment & qualification
On-site NP intake, bloodwork requisition, baseline measurements and consent. You're onboarded to the app and unlock your client courses.

Results review & treatment start
The NP reviews your labs. If you qualify, you receive your prescription, a titration schedule and injection teaching. You meet your certified trainer for a strength baseline.

First monthly follow-up
Side-effect check, tolerance, appetite and protein intake. The dose is adjusted as appropriate. Nutrition consult recommended.

Titrate & build strength
Monthly NP visits continue. Training progresses to protect lean mass. Repeat labs are ordered when indicated. Massage or physio can be added for recovery.

Between monthly visits, the app keeps you connected to the Nurse Practitioner and your support team.

Add-ons, any tier
Allied health sessions are available individually at $115 per session. Personal training, massage and other gym services can be booked through your location at member rates.
Payment & coverage
The program is private-pay and not covered by OHIP. GLP-1 medications are purchased separately at your pharmacy; many private insurance plans cover them, and allied services may be eligible under extended health benefits. Check your plan.
How we work alongside your physician
This program exists outside your relationship with your family physician. You don't need a referral, and your physician remains your primary care provider.
